NHTD & TBI Waiver Programs - Tailored Home Care and Support
HumanCare NY is proud to offer the NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury) waiver programs. These programs are designed to assist individuals with cognitive deficits and other specific needs, providing them with the necessary home and community support, as well as service coordination to improve their quality of life.
Comprehensive Care and Support Services
Under these waiver programs, we offer a variety of services, including additional training for caregivers, oversight and supervision to ensure the highest quality of care, and a comprehensive care plan tailored to the individual needs of each client.
Flexible and Responsive Care Solutions
Our approach is flexible, adapting to the changing needs of our clients. We provide up to two (12) hour shifts per day, ensuring that care is available when it's most needed. Our goal is to provide support that not only meets but exceeds the expectations of those we serve.
